If you live in the Kingsgate 3 and 4 neighborhood, we would love to have you on our team.

Signing up for the 2010 summer swim team is easy.

  1. Print and fill out the 2010 Midlakes Hold Harmless Form
  2. Print and fill out the 2010 Registration Form
  3. Print and fill out the Concussion Information Sheet
  4. Send the completed forms to the treasurer, Joli Hansson
